24/7 Access with My Kid’s Chart

Staying on top of your family’s health care can be a challenge, but our patient portal can help. Our portal is a work in progress.  Currently, you can have access to these features from your browser or smart phone:

  • Completed health assessment (physical) forms for daycare and school
  • Completed medicine administration forms for school/daycare (like asthma medications)
  • CHADIS questionnaires
  • Completed school excuse notes
  • Covid PCR test results
  • Payment options

Important Instructions

  • Always read and follow the label instructions.
  • Whenever possible, use the dosing device that comes with the product. Available options are syringes, droppers and medicine cups. Regular spoons are not reliable.

Acetaminophen (Tylenol)

  • Repeat dose every 4-6 hours as needed while symptoms last.
  • Do not give more than 5 doses in 24 hours.
  • Do not use with any other product containing Acetaminophen.

Ibuprofen (Advil, Motrin)

  • Not for use for children under 6 months of age.
  • Repeat dose every 6-8 hours as needed.
  • Do not give more than 4 doses in 24 hours.

Diphenhydramine (Benadryl)

  • We do not recommend using this medication for children under age 2 years, unless we specifically direct you to use it. 
  • Repeat dose every 6 hours as needed.
  • Children’s Benadryl Liquid: 12.5 mg/5 ml; 
  • Children’s Benadryl Fastmelts: Each fastmelt tablet contains 12.5 mg of Diphenhydramine HCL, which is the same as the chewable tablets.
  • Risk of side effects: May cause drowsiness or have a stimulant effect sometimes called parodoxical excitation. Use caution when driving or operating heavy machinery after dosing due to potential for sedation and decreased alertness. This is especially relevant for teen drivers.

CHADIS Pre-Visit Questionnaires

Save time, complete visit forms online

CHADIS is our online system that delivers developmental and health questionnaires directly to parents and families. The questionnaire results come directly to your doctor at Appleseed Pediatrics.

This information will help us address any concerns about your child’s health and development. Completing the questionnaire ahead of time also saves you time in the office

You should complete CHADIS questionnaires:

  • The very first time your child visits our office
  • Before every well-child visit
  • Before all ADHD and behavioral health visits.

Instructions

You can access CHADIS through our patient portal.

Please select well-child for all routine check-ups. For other visits, select the appropriate choice. If this will be your child’s very first time visiting our office, please be sure to select the New Patient visit type — either well-child for a check-up, or other visit for everything else.

Please call us if you have any problems registering or choosing the correct appointment type.

Telemedicine Visits

Your child’s next visit could take place in the convenience of your home via telemedicine.

See us virtually for

  • Rash, redness and skin infection
  • Cold, flu, and allergies
  • Medication refills
  • Behavioral health
  • Development screenings
  • Follow-up appointments
  • Pink eye
  • And more…

Please note

  • We reserve the right to request a patient come into the office to be seen.
  • Co-pays will apply to telemedicine visits and can be paid on the patient portal prior to your visit.
  • Policies are subject to change.

Tips for your telemedicine visit

  • We currently use Doximity for telemedicine visits. It is available for iOS and Android
  • You do not need to download any apps in advance. Before your telemedicine visit, we will let you know the instructions for how the visit will take place.
  • Good lighting is helpful. Place the light in front of you and your child, rather than behind you.
  • Please have your child’s recent weight and temperature.
  • We may need to review any medications your child is using, and verify your preferred pharmacy.

Privacy Considerations

Please understand that while this software is HIPAA-compliant, by choosing to do a virtual visit, you are agreeing to electronic communication of you or your child’s health information through audio, video, and/or photo technology.  While our software is secure, consider any privacy needs in your space as well.
